Every spin picks a random slice using the browser's built-in randomness source. Entries with the same weight have an equal chance every time; if you set a weight above 1 on an entry, it wins proportionally more often — that is a deliberate choice you control from the editor.
No — entries and customisation are per session. Refreshing the page or closing the tab resets the wheel to its starting state. Keep your list in a text file or spreadsheet if you need to reuse it, and paste it back in via the bulk import area.
